Steps To Complete Field Trip Approval Form

1.  Start by entering the trip information. All fields highlighed in yellow are required. Click Next to move forward once all required fields have been entered.

2.   If you miss a required field the tab name will be red. Please click the tab and fill any required fields that where missed. Scan any transportation related documents to a PDF file and upload.

3.   The final step once all required fields have been entered and transportation documents have been upload is to sign and click Submit. .
